POLIVKA - Ann M., nee Wald age 67 of Smithtown, NY formerly of North Olmsted, OH on March 5, 2014. Beloved Wife of Thomas. Loving Mother of Jeffrey (Gretchen) Polivka and Lauren (Matthew) LaBarbera. Adored Grandmother of Alexis & Thomas Polivka. Dear Sister of Larry Wald and the late Warren, & George.
Funeral Service was celebrated at The Branch Funeral Home. Interment followed at Sunset Memorial Park in N. Olmsted, Ohio.
